Steps for Scheduling
Step 1:
Call to 1-888-474-6532, 8:00 to 4:00 pm Monday to Friday or make an appointment online at least two weeks in advance to schedule your executive physical.
Step 2:
You will receive the GSH Executive Physical Examination Preparation Kit at least a week before your scheduled exam. The kit will include a preparation booklet with simple instructions and a description of what to expect on your executive physical examination and a complimentary GSH Executive Fitness Nike pants and jacket for you to wear during your testing.
Step 3:
On the day of your exam, the Executive Health Program Manager will greet you upon your arrival at the hospital and will escort you to our testing facility. The majority of the tests will be completed efficiently within one location. The testing process includes the following activities:
- Check-in and registration
- Collection of urine sample for urinalysis
- Blood draw for laboratory tests
- Break for snack (food will be provided)
- Radiology and Cardiology Tests (includes cardiac scoring, electrocardiogram, chest x-ray, carotid ultrasound, bone density scanning, and mammogram – optional test if female)
Step 4:
On that same day (or a later day if you wish), your GSH primary care physician will see you to conduct a complete physical examination (including a prostate exam for men and an optional pap smear for women), and review your test results.
A summary of your results will be provided to you in a personalized medical record book with electronic copies of your imaging tests on a compact disk (CD) and USB memory stick.
